24 hour cancellation notice required or you will be charged the full fee for the appointment.

You will get a confirmation email when you complete making the appointment. Check your spam filter if you did not see it.

Vaccinations and masks required.

You must be COVID-19 free and have NOT been tested for the virus in the last 14 days. 
You must be symptom free of anything resembling COVID-19.

Payment is due at the time of the appointment.

I am not accepting any insurance cases.
